Watch: Missed chances to protect murdered toddler

This was the 999-call from Lola James’ mother after she had suffered “catastrophic” brain injuries in July 2020.

A child practice review into the two-year-old’s death published on Thursday found “missed opportunities” to protect the toddler from her stepfather at her home in Haverfordwest, Pembrokeshire.

It pointed to a range of problems at multiple agencies, with processes not being followed.and an “overstretched” and “low morale” children services team.

Kyle Bevan, 32, of Haverfordwest, Pembrokeshire was given a life sentence while Sinead James, 30, got six years in jail for causing or allowing her daughter’s death.

Pembrokeshire Council said it takes its safeguarding duties “extremely seriously” and had developed an action plan.
