‘Serious concerns’ raised after Essex prisoner dies in Norfolk

Ms Goward, an area coroner for Norfolk, said she was not satisfied future deaths would be prevented.

She cited claims that documents had been altered, after being signed, and that documents were retrospectively completed.

She said that two prison officers, who gave evidence, said signatures of their names had not been written by them.

Ms Goward also said she had requested evidence from HM Prison Service, but that it had not been submitted to the court in time to be admitted.

She said: “There is a risk a full picture is not being received and could lead to the same thing happening.

“I will therefore release my report that there is a concern of future deaths if documents are not properly kept and if there is not full disclosure.”
