Reddit data reveals federal health workers hit a breaking point as anger surpasses pandemic levels

As shifting federal priorities, sweeping agency restructurings and deep funding cuts continue to reshape the American public health infrastructure, a new study reveals the toll these disruptions are taking on the federal workforce.

Published in the Journal of Medical Internet Research, researchers at Washington University in St. Louis analyzed nearly 45,000 public health-related posts from a prominent online forum for federal employees. They discovered that expressions of fear and anger skyrocketed in 2025, revealing a workforce under unprecedented psychological stress.

Using social media as a proxy for workforce sentiment

Traditional employee surveys often suffer from long administrative delays and low participation due to fear of workplace repercussions. To bypass these limitations and capture real-time, unfiltered employee sentiment, researchers from the Bursky School of Public Health turned to Reddit’s r/FedNews (FedNews) forum. The platform has become an increasingly vital anonymous outlet for federal workers sharing peer advice and voicing grievances.

The research team extracted 197,874 submissions between January 2020 and December 2025, filtering for 44,216 posts specifically mentioning key federal health programs and agencies, such as the Centers for Disease Control and Prevention (CDC) and Medicaid. Using advanced text-stemming and natural language processing (NLP) emotion lexicons, the team tracked how fear and anger metrics evolved alongside changing political landscapes.

Key findings: A workforce in crisis

The study’s data underscore a massive psychological shift among public health workers as discussions pivoted away from routine work arrangements to layoffs, budget cuts and agency downsizing:

• A dramatic surge in anger: Average anger scores in public health-related posts spiked by 63% in 2025 compared with 2024. Notably, the level of anger recorded in 2025 statistically surpassed the peak anger levels seen during the height of the COVID-19 pandemic in 2020.
• Rising waves of fear: Average fear scores rose by 32% over the same period, with workers expressing acute anxiety over sudden job insecurity, the abrupt termination of academic research contracts and the imminent threat of losing their own health care benefits.
• A shift in conversation: Topic modeling revealed that before 2025, forum discussions focused almost entirely on routine work matters, health benefits and telework. By 2025, the digital discourse had shifted completely to federal politics, mass layoffs and the severe physical symptoms of chronic workplace anxiety.

The hidden cost to public safety

The findings come at a time when federal funding priorities and the restructuring of public health agencies remain at the center of intense national debate. The study notes that a workforce operating under record-breaking levels of fear and anger faces immense barriers to maintaining optimal performance.

When federal employee morale collapses, the operational efficiency of critical national safety nets—including disease surveillance, food and drug safety, emergency preparedness and community health outreach—is inherently compromised.

The researchers caution that policymakers executing top-down agency overhauls must account for these immediate, measurable psychological costs, which threaten to compound the disruptions caused by budget cuts alone.
